I am an “out-of-network” provider. If you choose to seek reimbursement from your insurance carrier, I will provide a monthly “Superbill” that provides all of the information needed for you to file claims. I will make every effort to assist you in filing for insurance benefits. Many patients find that they are able to have the majority of the fee covered by their insurance company. If you would like to use out-of-network insurance benefits, you can call the phone number on the back of your insurance card and inquire about reimbursement. Questions you may want to ask include:

• What is my out-of-network mental health coverage?

• Is there a deductible that I need to meet in order to receive reimbursement?

• How many visits am I allowed per calendar year?
